I Paid Upfront for Medical Care or a Prescription. How Do I Submit a Claim?
As a participant in an Intrax internship or training program, you have most likely been enrolled in an insurance policy through Envisage. If you have not purchased your insurance through Intrax, you will need to contact your insurance provider for the details of your policy.
Intrax recommends that you keep your insurance ID card with you at all times so that you can present it to medical providers when you receive care. If you present the Insurance ID card during your visit to the hospital or doctor’s office, they can bill the insurance company directly.
If you receive medical care before receiving your insurance ID or if you misplace your card and had to pay upfront for medical care or prescriptions, you can submit a claim with the insurance provider.
Look at the top of your insurance ID card. It will either say Seven Corners or IMG.

Information about your insurance policy, including claim forms and a tool for finding an in-network medical provider, can be found at the Envisage Student Zone. There, you can find detailed information about your policy; search for in-network doctors, clinics, and hospitals; and print replacement insurance ID cards.
